To initiate a free 30-day trial supply for an appropriate patient, you should:

• Read the available Prescribing Information.
• Write a prescription for a 30-day supply of the applicable product. No substitutions are permitted.
• Refills are not required and there are no requirements to purchase any product or service to use the voucher. If you want your patient to continue taking the product beyond the free trial period, please write a separate prescription based on your recommended therapy.
• Select the appropriate check box on the voucher and fill in the dose.
• Give the valid signed prescription and the voucher to the patient along with the Medication Guide for the applicable product.
• Eligible patients can take the voucher and the prescription to any participating eligible retail pharmacy to receive their free 30-day trial supply.

The same free trial offer is available in different forms. For example, patients may receive the voucher from their doctor, or they may print the voucher themselves from the product Web site. Regardless of how many vouchers a patient receives or prints, patients may use only 1 voucher per product and receive 1 free trial 30-day supply of each applicable product 1 time before the expiration date printed on the voucher.

• The voucher is valid for 1 free 30-day trial supply per product.
• Limit 1 voucher per product per patient for the duration of the program.
• Valid for 1-time use per product only. Free trial offer is valid only for up to 1 30-day trial supply per applicable product. No purchase is necessary. Refills are not required.
• The voucher is not transferable. No substitutions are permitted. Cannot be combined with any other free trial, coupon, discount, prescription savings card, or other offer.
• The voucher is not insurance.
• Patient must be 18 years of age or older to redeem the voucher. Patient, pharmacist, and prescriber agree not to seek reimbursement for all or any part of the benefit received by the patient through this offer.
